The Treasurers Office is located on the first floor of the Main Hospital. Patients should park in the “A” parking lot just west of the main entrance. Proceed down the main hallway and turn left before the third stairwell. The Treasurers Office is the third/last door on the left had side, across from the Admissions & Dispositions Office. Look for the sign that says "Treasurers Office".

We always reccommend putting the Patient IEN (Internal Entry Number), found on the invoice directly above the patients name, in the Memo comment on the check or money order for faster processing.
All patients will be asked to at least annually update their information on what other health insurance they have (if any) utilizing DD FORM 2569.
There are no payments required for outpatient services provided to eligible beneficiaries. Active duty service members and their family members (enrolled in Tricare Prime) and retired service members incur no cost for inpatient care. For all other eligible beneficiaries, patients will be billed the current Family Member Rate (FMR) per day they are admitted. Family Member Rate (FMR) for Fiscal Year 2017 is $18.20 per day for family members. These charges are due and payable upon discharge in the Treasury Office next to Admissions & Dispositions (A&D).
The Third Party Collection Program gives Evans Army Community Hosptial (EACH) the ability to provide our patients with the essential equipment and health care staff, that will ultimately enhance the overall quality of care.
Our patients can help make this a reality by completing a DD FORM 2569 and returning it to us by email or in person at the next visit. Our hospital receives 100% of the funds reimbursed by civilian health insurance companies to fund additional services, upgrade the facilities and equipment, and add programs for our patients. Patients are not liable for any costs not paid by civilian insurance to include deductibles cost-shares and co-pays.
Everyone benefits from completing the DD FORM 2569! Patients should remember to update this form every year even if they do not have other health insurance.
