Mountain Post Medical Home and Premier Medical Home set to close

U.S. Army Medical Department Activity-Fort Carson Public Affairs Office

Evans Army Community Hospital will close Mountain Post Medical Home and Premier Medical Home in Colorado Springs, Colorado, within the next several months.

Due to unforeseen circumstances, the lease agreement for Mountain Post Medical Home was unable to be extended, and the lease agreement for Premier Medical Home will expire this summer.

Both clinics will consolidate within Iron Horse Family Medicine Clinic at Evans Army Community Hospital at Fort Carson. The clinic healthcare teams will continue to provide care to their current patients to ensure the integrity of care and maintain established relationships. Additional clinics and services are also available at the hospital.

“Your team of healthcare professionals will continue to partner with you to develop a comprehensive healthcare plan,” said Col. Eric S. Edwards, Evans Army Community Hospital commander. “It is our intent to keep you with the same, trusted team you’ve come to know, at a location that has substantially more specialty services and capabilities.”

Patients assigned to Mountain Post Medical Home and Premier Medical Home can expect to receive notice of these changes from Health Net Federal Services and Evans Army Community Hospital.
