Self-Care Program
Evans Army Community Hospital
(EACH) provides Soldiers,
their Family Members, and Tricare Beneficiaries access to over-the-counter (OTC)
medications. For a list of available medications, click
HERE.
Required: Self-Care Card
Self-Care Program
cards issued by other military treatment facilities are honored at
Evans Army Community Hospital pharmacies.
To obtain a Self-Care
Program card, education regarding self-care principles and
medication is required. Self-Care Program classes are held every third
Wednesday of the month in the Cochrane Hall, in the Evans
Hospital's second floor, from 1:00 - 1:30 PM (no reservation
necessary), or may be coordinated for groups through the Army Public
Health Nursing by calling 719.526.2939. An additional option
is to view the online presentation
(Self-Care
Program Online Presentation),
print and present the last slide of the presentation to ANY of the
Evans Army Community Hospital pharmacies or bring it to the
Preventive Medicine Department's front desk (building 2059,
outside Evans Hospital West entrance) to receive a Self-Care Program
card.
After obtaining your Self-Care
Program card,
present your
card along with the
Self-Care Medication Request Form
at ANY
Evans Army Community Hospital pharmacies, to include the Premier
Army Health Clinic off-post, to obtain up to four medications per
family per month.
Limited to
ONLY four medications per family per month.
