Nutrition Care Division Outpatient Nutrition Clinic

   Individual Consultations:

·         No consult is required for general nutrition, sports nutrition, vegetarian diets, cholesterol education, weight management, and prenatal nutrition.

·         Initial appointments for all other diagnoses require a referral from your primary care provider.

·         Individual appointments are seen in the Nutrition Care Division on the 1st floor at EACH, Room 1919.

   Group Classes:

·         Active Duty Weight Control, Taping Certification and Cholesterol Education Classes are all taught at EACH. These classes do not require a referral and you can sign up by calling 457-2273 or  526-7290 

     Required Nutrition Education for Bariatric Surgery:

·         You need a referral from your primary care physician (to determine if you are a candidate for bariatric surgery) then call 457-2273 to schedule yourself in a class.

·         The two-part Gastric Bypass Education Program is designed to provide patients with exercise skills and nutrition knowledge to ensure successful surgery outcomes.

·         The class is divided into 2 sessions of which both must be attended to receive credit.  The classes are held in the Nutrition Care Classroom.    Please call the Nutrition Care Division @ 526-7290 to get scheduled for the class.          Space is limited.

Gastric Bypass Support Group meets the 3rd Wednesday of each month at 1800 hours Kit Carson Community Center        (off of Harr Avenue near Gate 2)

Diabetes Education Program:

·         The class is for all adult patients diagnosed with hyperglycemia, type 1 or type 2 diabetes. A basic explanation of diabetes including medications, foot care, nutrition and exercise is discussed, based on the American Diabetes Association and the American Dietetic Association Guidelines, taught by the multidisciplinary medical team.

·         Includes an introduction to dietary management of diabetes and carbohydrate counting.

·         The program includes a 4 part class scheduled through the Disease Management Section on the                  2nd floor of the SFCC.

·         Clients receive an individualized meal plan based on the ADA Exchange System. With an emphasis on carbohydrate counting, patients are taught how to use a meal pattern and use food labels to plan meals. Attendance at the basic diabetes class is required for admission to the meal planning class.

·         Contact Disease Management at 526-7022 to sign up for the class.
