Queue	Neurology (WRC)
	AD Only:  referred ONLY by Dr. Covas-Maldonado, PA Kremler, and PA Johnson. ALL ADSM referrals are generated by
	the SRC- DHC only
	PRIME ADULTS ONLY - referred by PCM (i.e. non combat TBI)
Beneficiary Type	For those referrals for AD and Prime referred by a provider other than those listed above (SRC-DHC for ADSMs or
	PCM for Prime Adults) place in the appropriate WRC Queue for clinic review.
	Sleep Eval: AD ONLY: referred by WRC providers (Craig, Foust, Grogan, Martin, Lucero, Olin, Finley, and Souvignier) and
	Dr Olin, Dr. Finley and Dr Pagdan- appoint to Dr Pazdan.
	AGE RESTRICTION: 18+
	CRC/PAS
	Initial consults for EACH PRIME (18+) will be appointed with Ms. Oly and PA Lucero ONLY.
	(If there is no access with Ms Oly or Mr Lucero, then patients may be booked with Dr Craig or Dr Pazdan).
Reviewer/Book	Referrals from Dr Finley or Dr Olin (Optometry) send referral to WRC OT Queue for clinic review/scheduling.
	Dr Langford sees WRC/TBI patients only after consultation and discussion with clinic personnel.
	Referrals for WRC ST and WRC Neuropsych Testing must be generated by WRC Providers.
	Referrals for WRC PT can be generated by any WRC provider (Oly, Lucero, Craig, Pazdan, Grogan, Foust or Martin,
	Pagdan or Lanford), WTU providers (Dr. Solerno, Dr. Beitz, or Dr. Meyer), Pain Management providers (Dr Baer
	and Dr Meiners) or EACH Audiology providers.
	Referrals for WRC OT can be generated by any WRC provider (Oly, Lucero, Craig, Pazdan, Souvignier, Foust, Martin,
	Pagdan, or Langford).
Reviewer/Book	Referrals for Vestibular Rehab (i.e. dizziness, vertigo, balance and vestibular) may be entered by any provider. Place in
	WRC PT Queue for clinic review.
	For WRC PT Referrals- appoint to WRC PT queue.
	For WRC OT Referrals- appoint to WRC OT queue.
	For WRC ST Referrals- CLINIC REVIEW and BOOK.
	For WRC Neuropsych Testing- appoint to Neuro WRC and indicate referral is for NP Testing.
	CLINIC REVIEW AND BOOK INCLUDES THE FOLLOWING:
	PCM referrals for non WRC/TBI and Concussion patients:  Place in Neurology Clinic queue
	PCM referrals for non WRC/TBI patients:  PT vestibular rehab, vertigo, dizziness, or balance evals - Place in Neuro WRC
	PT queue.
	Form 8045:  Place in Neuro WRC queue
	MEB Evaluations:  Place in Neuro WRC queue.
Reviewer/Book	Sleep Gym:  All ADSM's (other than WTU SM's) referred for Sleep GYM are ONLY to be referred by WRC personnel
	(Craig, Pagdan, Pazdan, Oly, Lucero, Martin, Foust, Souvignier, Langford) and Dr. Olin.
	Sleep Gym:  WTU soldiers- Place in WTU OT queue for review/book.
	TDRL Evaluation:  Place in Neuro WRC queue.

	Referrals requesting 2nd opinions will not be scheduled as enhanced (will be booked in EST appt type).
