The Warrior Family Medicine Clinic is located on the second floor of the main hospital. Patients can park on the West side, parking lot “A”, closer to the mountains. Using the west entrance, proceed to the first stair case and go up to the second floor via the stairs or elevator. Once on the second floor, proceed across the bridge and take a left. Your clinic will be on the right hand side of the walkway. Alternatively, patients can use parking lot “F” and enter the main hospital from the breezeway courtyard. Using this entrance, turn right, proceed to the first stair case and go up to the second floor via the stairs or elevator. Once on the second floor, proceed across the bridge and take a right. Your clinic will be on the left hand side of the walkway.

Your Patient Centered Medical Home (PCMH) provides accessible, family-centered, comprehensive care. PCMH focuses on the whole person! That means prevention, early intervention and management of health! We are NOT patient centered, unless YOU say we are patient centered!

You will receive a letter from UnitedHealthcare within 10 working days.

What information about my healthcare can I get by telephone?
