The Clinic is located on the Southwest Corner of the intersection of Union Boulevard and Austin Bluffs Parkway.

Premier Medical Home is the first Army community-based primary care clinic in Colorado Springs. Premier Medical Home applies a multidisciplinary approach to deliver comprehensive primary care. Each patient will partner with a team of healthcare providers – physicians, nurses, behavioral health professionals, pharmacists, and others to develop a comprehensive, personal healthcare plan.
Premier Medical Home staff provides care to enrolled adult and pediatric patients in a Patient Centered Medical Home environment. Within the clinic there are two similar teams. Each team includes a mix of Physicians, Nurse Practitioners, or Physician Assistants; and nurses, a Clinical Psychologist and administrative personnel working together to deliver care for military family member or retiree families. Our goal is to maximize the health of our patients and support their families.

Who can be seen here?
Active Duty Family Members, Tricare Prime Retirees up to age 62, Infants, Adolescents, and Children. Patients 10 years of age and greater must present a valid military identification card to obtain medical services. Please ensure all patients 10 years of age and greater are in possession of a valid military identification card before arriving at the clinic.

Walk-in Services 8:00 am - 11:00 am and 1:00 pm - 3:00 pm

Commonly asked Questions and Answers
What is the Nurse advice line?
o Overview: The TRICARE Nurse Advice Line will provide 24 hour/day, 7 day/week nurse triage, advice and MTF appointing for acute medical issues. The NAL goals are to recapture care to the MTFs, to reduce unnecessary ED/UC utilization, to improve the patient’s continuous healthcare relationship with his/her MTF and PCM, and to maximize patient satisfaction.
o Eligibility: All TRICARE beneficiaries (NAL staff will verify thru DEERS).
o Patients with MTF-only privileges ie Dependent parents, are not eligible to use the NAL
o Local Care Line access to National NAL: 719-524-CARE (2273), Option 3
o This is the same way our beneficiaries are currently accessing the:
 Local nurse advice line during regular duty hours.
 Local PCM on call service after regular duty hours and on weekends.

What forms will I need for Schools or Day Care?
o Many school/daycare forms can be handled by speaking to your child's care team.
o Patients 10 of age and greater must present a valid military identification card to obtain medical services. Please ensure all patients 10 years of age and greater are in possession of a valid military identification card before arriving at the clinic.

How do I refill my medication?
• If you have refills remaining on the prescription you can call
719-526-4081 or click here
to request refills. Please note Premier Pharmacy is not a refill pharmacy and refills generally take three days to be ready for pickup.
• If you have no refills remaining please send a message on relay health or call the PAS service at 524-2273 to request a medication renewal. Please give us three business days to process your prescription renewal.

What should I expect when I get a referral?
o If you are assigned to a military facility you should receive a phone call from the specialty clinic to schedule your appointment.
o If you are assigned to a network provider you should receive a letter within 7-10 days with your referral information. You can call Tricare at 1-877-988-9378 for information about your referral or to request a change in network provider.

How do I get refills if I have a pain contract?
o This should be specified in your contract and you should have received a copy of this contract for your records. Most patients will need an appointment for refills of narcotic/opiate pain medication.

What is Relay Health? How do I register for relay health?
o Relay Health is a secure e mail system between you and Premier Medical Home. This allows you to email your provider with questions or needs, such as medication renewals. It also allows us to email you with responses, results or nurse advice. You can click here or call our office to be sent an invitation. If you have come from another facility where you had a relay health account, please let us know and we can transfer your account to our clinic.

What is Tricare Online? How do I register for this?
o Tricare online allows you to see your referrals and labs and to make your appointments online. You can go to to register for your account.
• Suture/Staple removal
o Appointment to be made for services for provider to assess the wound. May send Relay Health message or call to discuss with nurse.

Other information important or critical for patients to know
• After hours, weekends, holidays contact the provider on call by calling the patient appointing line 719-524-2273 and follow prompts.
• Pharmacy hours: Mon thru Fri 8:30 a.m. —12:30 and 1:00—5:00 p.m.
• Laboratory hours: Mon thru Fri 7:30 —11:30 a.m. and 1:00—3:50 p.m.

Premier Extended Hours Clinic
Evans Army Community Hospital announces the opening of its new Premier Extended Hours Clinic at the Colorado Springs based Premier Medical Home. The clinic is open Monday through Friday from 4:00 p.m. to 9:00 p.m. except federal holidays. Patients must have scheduled appointments; walk-in appointments are not currently available. The Extended Hours Clinic will provide medical services to active duty military personnel, active duty family members and TRICARE Prime beneficiaries. TRICARE Standard patients are not eligible. Patients must be at least 18 years or older during the onset -- we plan to add pediatric services in the future. Upon arrival for their appointments, patients must present a valid military identification card. The clinic also provides basic laboratory and pharmacy services, however, it does not provide emergency services, does not offer pharmacy refills, specialty care or radiology services. The Premier Extended Hours Clinic is located at 3920 N. Union Blvd, Suite 100, in the Premier Medical Plaza at the intersection of N. Union Blvd and Austin Bluffs Pkwy near the University of Colorado Colorado Springs campus. Patients may schedule an appointment at the clinic by phoning the Access to Care Line at (719) 526-CARE (2273).
