Outpatient Medical Records

PAD HOME
For all requests of Military Outpatient Records from your 'losing' Military Treatment Facility (MTF) fill out a DA form 877 at the Outpatient Records Office

The Outpatient Medical Records department is located in the Soldier Family Care Center (SFCC) on the 1st floor (lower level) Building 7503 (east-side) Room # 1500

Hours of Operation Monday-Friday 0730-1800 (Thursday 0730-1230 only)

When In & Out processing the installation please provide 2 copies of orders. If you are going to a remote assignment with family members please allow 5-7 working days to get your records updated (printing of all electronic records).
