Congratulations
on the birth (or impending birth) of your baby girl or boy. While your newborn
is in the hospital, you will be given a birth certificate worksheet to
complete. This will register the baby with the State of Colorado. However, you
must go to the El Paso County Public Health Office to get the actual birth certificate.

Any changes that are needed on the birth certificate must be made to
Evans Army Community Hospital (EACH) Birth Certificate Registrar at 526-7278

Parents will also be given the opportunity to request a social security card for
their newborn through Evans Army Community Hospital. You may alternately
obtain a social security card through the Social Security Administration,
however, you should not try to obtain one from both places (this could establish
two social security numbers). Social Security cards take approximately one
month.
With the Evans Army Community Hospital (EACH) Certificate of Birth, you will need to go to the ID card office to complete your
child's DEERS enrollment. You may enroll your child at the following locations:
