Self-Referral   Both.  If a soldier is 'self referring', you will need to build a referral.

Special Instructions for CLERKS:  
 
  All requests for special evaluations, school, recruiting , chapter, and any command directed evals are clinic book only.
  Patients not having been seen in the clinic for nine months or more, book in a SPEC with a first available provider.
  If a patient has “no showed” 2 times in the past 6 months, DO NOT REBOOK.  Transfer the patient to the clinic.
  Requests for medication refill should be transferred to the clinic.
  Any patient that requests to reschedule an appointment MUST be rescheduled with the same provider that they were initially seen by. 
  Do not book MEBs.
  Do not book Specialty Schools Evals, Command Directed Evals, Chapter Evals, Retention Assessments (fitness for duty), and profile
       initiation or removals.  Transfer these to the clinic.
 
Follow-ups
  When booking an EST, simply annotate that this is a follow-up.  Do NOT ask what this is follow-up to or what it is for.
  All EST appointments are to be booked in 60 minute slots unless the patient asks for a specific appointment time as requested by
       their provider.
 
Referral Lifespan:
  Patients NOT having been seen in the clinic for 9 months or more, book in a SPEC with a first available provider.
  Referrals for patients routinely being seen by the clinic DO NOT expire.
 
Community Behavioral Health Service: 
  Book using the Psychology (Community Behavioral Health Service)  protocol.
 
"Booking Queue" Instructions:  Referrals and consults
  Consults in the booking queue for SPEC or EST appointments will state which provider and appointment duration to book to.
 
  Call patient and offer the next available slot for the specified provider.
 
  If you are unable to contact a patient after the third attempt, the referral is to be returned to the ordering provider by a Team Lead
       or Supervisor. 
 
  If a patient refuses to book an appointment, the referral is to be returned to the ordering provider by a Team Lead or Supervisor.
 
  If a provider is NOT specified or the following statement is shown:  "Patient may be booked to any available provider per protocol."
        offer the first available with any provider listed below.
 
  Providers that may be booked to are:     Butler                    Kalhorn
                                                                         Campbell              Martinez
                                                                         Coshignano           Schurtleff
                                                                         Husted                  Wilkinson
 
    If there are NO SPEC appointments within two weeks, have a Team Lead annotate this on the referral and
     return it to the REVIEW QUEUE.
Appointment Reminder:
  Once the patient agrees to a specific appointment and they are being booked, print their future appointment as follows:
       1.  Select:  Remind patient of other appointments/Wait List requests? No//”
                  It is defaulted to NO.  Change to Y for YES and press ENTER.  It displays all pending patient appointments.
       2.  You can “Select DEVICE:" to print to by typing in “MEDS1” and pressing ENTER.  This prints the reminder at the clinic.
 
  IF YOU HAPPEN TO FORGET TO PRINT THE REMINDER, you can also print one by doing the following:
      From the 'Main' screen in CHCS:
                                                                 a.  Type “F” at the (F)uture/Past Appts prompts.
                                                                 b.  Press ENTER,  and then ENTER again.
                                                                 c.  You will be at the “Select DEVICE:" screen.  Type:  MEDS1
                                                                 d.  Press ENTER to print the reminder at the clinic.
 
 
Special Instructions for Lack of Availability:
  If there are no appointments available, transfer the patient to clinic for assistance:  222#
