Special Instructions for Patients:
TO BE READ VERBATIM TO THE PATIENT
 
  Children must be supervised at all times.
  Temporary profiles may be obtained from TMC until appointment date.
X-Ray / MRI:
  Whenever possible, bring relevant x-rays, MRI disks, or any reports from other medical facilities to the visit with you to
       your appointment.
  If you had x-rays done at USAFA or Peterson, you must bring a CD with those x-rays to your appointment.
 
 
Special Instructions for CLERKS:  
Provider Restrictions
 
  Active Duty Army must be seen in EACH podiatry unless referred out by EACH podiatry.
  Profile extension requests should be booked in follow up (EST) appoinments.
  Patients seen for INGROWN TOENAILS must be booked in two joined 20 min appointments.
 
Follow-Up Appointments:
  Book follow-up with the same provider patient originally saw.
  If the original provider has NO appointments when looking out at least 45 days, then book with another provider.
 
PA Nolen:
  Patients may be referred to provider Mechanik by PA Nolen if the patient states that they have been referred to Mechanik.
 
Referrals requesting “Second Opinions”
  Referrals requesting a “second opinion” are ONLY to be booked with physicians - NO PA's (Joanne Nolen).
 
Referral Lifespan:
  Podiatry referrals are good for a one year period.
  Patients will need to see their PCM for a new referral beyond the one year period.
 
 
Special Instructions for Lack of Availability:
  If there are no appointments available, transfer the patient to clinic for assistance:  227#
