| Physicals Phase 1: |
►All service members
require labs be completed PRIOR to appt.
Labs are to be drawn at the SRP Site, Bldg 1042, room 124,
between

| 0730-0900 at least 24
hrs PRIOR to their appt. |
| ► Fasting is required for 8-10 hours prior to lab work. Water is allowed. |
| ► If service members fail to complete their lab work, prior to
their appt, their appt will be rescheduled. |

| Physicals Phase 2: |
| ► All requests for Phase 2 physicals are to be booked FIRST
with the patient’s PCM following that clinic’s protocol. |
| ► RETIREMENT: For Phase
2, join TWO back-to-back SPEC appointments. |

| Chapter Physicals: |
| ► Chapter physicals must be scheduled by the soldier’s 1st
Sergeant directly with the clinic. |
| ► Exception: We may book
ETS, Retirement, and Pregnancy Chapter Physicals. |

| ► For Phase 2, join TWO back-to-back SPEC appointments. |

| Periodic Health Assessments: (PHAs) |
