OB / GYN Provider Listing
(click on link above)

Ask patient for their last menstrual period.  Annotate this on the booking comment section.  If unknown or unsure, annotate this as well.
Schedule the patient with the same provider they have been seeing whenever possible.
Look at other providers as a last resort by modifying referral and removing provider name.
Please remember that a patient may be seen within the week of the recommended appointment date.
If Patient requests an OB physical, transfer call to clinic:  231#
Ask the patient how many weeks pregnant they are.  Annotate this on the booking in the comment section.  If the patient states
that they are unsure or do not know, make note of that as well.

NPCL Instructions:
Patients DO NOT need a positive pregnancy test (MTF or otherwise) to be booked.
DO NOT BOOK NPCL APPT FOR THE SAME DAY the patient calls.   The clinic requires 1 day for preparation.
FIRST OB appt (NPCL) is Registration and is booked with a NURSE, not an MD - Book at 8 weeks-10 weeks.
Late entry (after 10 weeks) or TRANSFER-IN patients; transfer call to clinic at:  231#
“REASON FOR APPOINTMENT” MUST ONLY SHOW – The first date of LAST MENSTRUAL PERIOD (LMP) and your
initials or if LMP is unknown, enter the following: "LMP is UNK, PT states they are [X] weeks pregnant as of [DATE]" and
        your initials.
Do NOT add any other information such as “Positive Pregnancy Test.”  For NPCL booking - NOTHING ELSE IS REQUIRED!

If Pt doesn’t know either LMP or X weeks pregnant, book in first available NPCL. Prenatal class (must be taken prior to any
      WELL Appointment)
Acute Issues:
Pregnant patients with ACUTE issues, transfer call to Evan's OB clinic to speak with a Triage Nurse:  231#
Acute issues include BLEEDING, PAIN, NAUSEA, AND VOMITING.
Any patient that feels they need to be seen immediately should be transferred.
These patients DO NOT need to have attended the Registration Class or have had a physical.

Post-Partum PAP:
For postpartum paps look first to book into detail coded PP/PAP slot.  If no access then book into a PAP slot.
Be sure to annotate PP PAP in the reason for appt.
BreastFeeding Group Class:
  Offer the GRP class first.
If they absolutely cannot attend the GRP class or insist on an individual class, then book a WELL appointment.
Located in front part of OB clinic classroom.

NEVER ask a patient if they are complicated or routine.
Ask if they have been seeing a midwife or a doctor.
Midwife = book in a OB detail code = Routine pregnancy.
