ACUTE BPAPS 30   Patients who have been discharged from inpatient hospital  within the past 30 days
  and need follow-up for blood clots, anti-coagulant or blood thinning medications.  
EST BPAPS 30   All patients (not discharged from inpatient hospital) who have blood clots, or need
  anti-coagulant or blood thinning medications.
Special Instructions for Patients:
TO BE READ VERBATIM TO THE PATIENT
General:
  Arrive 15 minutes prior to start time.
  No children in class, except those with a scheduled appointment.
  Spouses may attend with patient.
  Bring all medications to class.
 
Diabetic Education Patients:
  Bring glucometer to class.
  Located at the Health and Wellness Center (HAWC).  Patients can call for directions:  333-3733
  This is a two part class.  Both must be attended in the same month.  Classes are held sequentially on the 1st & 2nd Thurs, 0800-1200.
 
Asthma Education Patients:
  Bring peak flow meter, spacer and all inhalers to class.
  10th MDG at USAFA Hospital, Allergy/Immunizations Clinic on 1st floor.
  Classes 3rd Wednesday of each month, 1330-1500.
 
High Blood Pressure and Cholesterol Education Patients (A Healthy Heart):
  Classes are held 3rd Tues of each month  at the HAWC.  Patients can call for directions:  333-3733
 
Mon and Wed Coumadin Clinic Patients:  (USAFA enrolled Prime and T-Plus ONLY)

  Check-in at A Family Medicine Clinic front desk.
  If a patient has a question, they may call 333-5883 to leave a message.
  This is a self-referral clinic.  If the patient does not have a referral in the system, create one. 
(There is NO expiration of any existing referral for Coumadin patients.)

Special Instructions for Lack of Availability:
Before transferring, be sure to push the dates out when looking for access.
If there are no appointments available, transfer the patient to clinic for assistance:  17#
If there are no appointments available in the Coumadin Clinic, transfer the patient to  333-5883.
