| ► Patients may be scheduled for A) GRP classes only, B) GRP classes THEN a SPEC, or C) a SPEC then the GRP classes. | |||||||||||||||
| Common examples (these are examples for illustration purposes ONLY): | |||||||||||||||
| Pre-Diabetes may have the GRP classes only. | |||||||||||||||
| Type II Diabetics may have the GRP classes then a SPEC | |||||||||||||||
| Bottom line – FOLLOW the instructions on the consult! | |||||||||||||||
| GRP Diabetes or Pre-Diabetes Classes 1 through 4: | |||||||||||||||
| ► Group classes may be booked anytime within the ONE YEAR lifespan of the referral. | |||||||||||||||
| ► Book all four classes for the patient to ensure placement in class. | |||||||||||||||
| ► If patient misses, no-shows or cancels any one class, they MUST reschedule the missed class before | |||||||||||||||
| continuing on with the class sequence. | |||||||||||||||
| Referral Lifespan: | |||||||||||||||
| ► If an existing patient is seen anytime during a 12-month period then a new referral is NOT needed. | |||||||||||||||
| ► A new referral is NOT needed if the patient continues to be seen routinely after the 12-month period. | |||||||||||||||
| ► If a Doctor gives a patient a new referral and it has been under a 12-month period then a SPEC appointment is NOT necessary, use EST. | |||||||||||||||
| ► If an existing patient goes OVER a 12-month period between visits, a new referral IS needed along with a SPEC appointment. | |||||||||||||||
