Special Instructions for Patients:
TO BE READ VERBATIM TO THE PATIENT
  Only children with booked appointments allowed.
  Bring list of all meds and dosages to appointment.  Diabetics should bring glucometer and written log of readings.  Asthma patients should
       bring peak flow meter, spacer and all inhalers.
  Inform patient that if they miss, no-show or cancel any one class, they MUST reschedule the missed class before continuing on with
      the class sequence.

Age Restrictions:
  Diabetes:  Only ages 17 years and up.
  Asthma:    Only ages 6 years and up.
Class Requirements:
  Classes will be scheduled per the instructions on the referral.
  Group classes must be attended in sequence: Class 1=EVAL, Class 2= COUNS, Class 3=EDU, Class 4=DM
  Patients may be scheduled for A)  GRP classes only,  B)  GRP classes THEN a SPEC,  or  C)  a SPEC then the GRP classes.
      Common examples (these are examples for illustration purposes ONLY):
           Pre-Diabetes may have the GRP classes only.
           Type II Diabetics may have the GRP classes then a SPEC
Bottom line – FOLLOW the instructions on the consult!
GRP Diabetes or Pre-Diabetes Classes 1 through 4:
  Group classes may be booked anytime within the ONE YEAR lifespan of the referral.
  Book all four classes for the patient to ensure placement in class.
  If patient misses, no-shows or cancels any one class, they MUST reschedule the missed class before
      continuing on with the class sequence.
Referral Lifespan:
  If an existing patient is seen anytime during a 12-month period then a new referral is NOT needed.
  A new referral is NOT needed if the patient continues to be seen routinely after the 12-month period.
  If a Doctor gives a patient a new referral and it has been under a 12-month period then a SPEC appointment is NOT necessary, use EST.
  If an existing patient goes OVER a 12-month period between visits, a new referral IS needed along with a SPEC appointment.
