  Have them call Cadet Standards at 333-0533.

    Chu:  PAP and GYN issues or overflow ACUTE ONLY -  patients between 17 and 26 years old ONLY
    Witkop:  PAP and GYN issues ONLY
    Campbell:  Sees only Intercollegiate (IC) Athletes for muscle, joint issues, strains, sprains or broken bones.
    Webber:  Book patients assigned to Webber to ANY available Cadet Medicine provider.
   
  If they are a cadet in a flying program and want “to return to fly” book using A Flight Medicine instructions.
  If they are an Intercollegiate (IC) Athlete book with Campbell as first choice.
   
    Book supplemental providers first whenever possible per restrictions.

  ACUTE booking priority
   
    1. Using provider restrictions, book with 'Supplemental Providers' first.
    2. Any Cadet Medicine Primary Care Provider 
    3. Transfer to the clinic.  
    4. If there is no answer, tell Cadets - 'walk-in' to the clinic.  
        For Cadet STAFF or other ACTIVE DUTY:
  Nurse Advice Line 06:00 - 14:15 / Urgent Care Clinic (UCC)
    5.  After going through ALL available options above, and the patient requesting to BE SEEN TODAY, please tell the patient:
  "I apologize, but I have no appointments available.  Please hold while I transfer you to an Advice Nurse for further assistance."
    6.  Attempt to perform a WARM transfer to the Nurse Advice Line:  524-8669
    7.  If the Nurse Advice Line is busy (WAIT 45 SECONDS to connect), or AUTOMATICALLY after 14:15,
          proceed to the Urgent Care booking as follows:
 
                a)  Click on the following link for the Urgent Care Clinic (UCC) guidelines
                  b)  Enter the UCC referral as instructed
  NON-ACUTE booking priority

    PHAs for NON-FLIERS ONLY

    Self refer (NOTE: Search for these providers in A PHA Clinic.  Providers are Binion, Sims, Searle, Blanco, and McGough.
    Book out NO MORE than 3 weeks (21 days) out.  (A Web-HA is good for only 30 days)
    If for some reason the patient MUST book out further, inform them that they will need to complete a new Web-HA
        prior to their appointment.   (See instructions below)
    If the patient has further questions, transfer them to 333-0042 for assistance.
  PHA Instructions for Patients:
               All patients must arrive 15 minutes early.
            Patients should have completed a Web-HA (Health Assesment) prior to their appointment.
               Patients unable to access Web-HA online, need to report 30 mins early to complete a Web-HA in the clinic.
            Contact lenses must NOT be worn.  Patients should wear their glasses.
               No cell phones are allowed to be used at any time during your appointment.
            If a cancellation should be necessary, it should be done at least two hours in advance of the scheduled appointment time.
               Children, under the age of 12, cannot be left unattended in the clinic waiting area.
 
          Patients should check in at the Family Medicine Clinic for their PHA appointment.
   
    Inter-collegiate Athletes:
    Book strains, sprains or broken bones with Campbell as first option. If no access, follow protocol as normal.
    Muscle/joint issues book with Campbell as first option.  If no access, transfer call to Cadet Physical Therapy Clinic:  333-5055
   
    Muscle - Joint - Strain - Sprain:
    Book with Cadet PT.  If no access, follow protocol as normal.

    Elbow, Forearm, Wrist, Hand injuries:
    Book with Cadet OT.  If no access, follow protocol as normal.
    See 'Inter-collegiate Athlete' for additional instructions.
   
    Broken Bones:
        For patients stating: “I think I may have broken my____________! 
    Book same day ACUTE in the Cadet Medicine Clinic.
    IF there is no same day ACUTE, have the patient walk-in to the Cadet Physical Therapy Clinic.
    In ALL cases, if patient feels that it is appropriate they may visit the nearest emergency room or call 911.
    If patient insists, transfer to clinic.

    Other Service Branch Cadet Instructions:
    Exchange students from other service schools i.e. (Navy Midshipmen, West Point Cadets, etc.) must be Mini-Reg'd, but may be
        scheduled as if they were USAFA Cadets.  The same rules will apply.
   
    Women's Health Issue/PAP Booking:
    Book with Case, Chu, or Witkop.
    Book all GYN follow-ups with Witkop in the WELL GYN slot.  If there is no access follow normal booking priorities.
   
    Mental / Behavioral Requests:
    Any requests for an appointment for social / behavioral / mental health, transfer the call to the Mental Health Clinic at 333-5177.

  TO BE READ VERBATIM TO THE PATIENT
         CADET PHYSICAL and CADET OCCUPATIONAL THERAPY:
    Report to the clinic 10 minutes before your appointment time.
    Patients with hip, knee, or lower leg injuries need to wear or bring shorts to change into for their appointment.
    Female patients being seen for shoulders or upper back should bring a sports bra or tank top to wear for the examination.
    Bring your ID card.

  FLIER/JUMPER  Flight Physical:
               Book with Flight Surgeon under: A FLIGHT MEDICINE
     A PHA is a sub-component of the Flight Physical.  ANY flier/Jumper requesting ONLY a PHA should be transferred to the 
   clinic.  The same rule applies for a request for a 1042 form completion.
  FLIER/JUMPER  Flight Physical Instructions for Patients:
               All patients must arrive 15 minutes early.
            Patients should have completed a Web-HA (Health Assesment) prior to their appointment.
               Patients unable to access Web-HA online, need to report 30 mins early to complete a Web-HA in the clinic.
            Contact lenses must NOT be worn.  Patients should wear their glasses (if applicable).
               No cell phones are allowed to be used at any time during your appointment.
            If a cancellation should be necessary, it should be done at least two hours in advance of the scheduled appointment time.

  Return to Flight/Jump Status:
    Book with Flight Surgeons only in GROUP appointment.
         If there are NO available GRP slot then an EST may be used.  This is not to be used only for patient convienience.

    If there are no appointments available, request the patient walk-in to Physical Therapy.
