Clinic Location:   2nd floor of the Soldier-Family Care Clinic on the east side of the main hospital.  
  Parking on the east side of the SFCC is most convenient (Parking lot G or F).

TO BE READ VERBATIM TO THE PATIENT
  Children cannot be left unattended in the clinic waiting area. 
  Any patient 10 yrs and above must present their ID card at the time of signing in to appt.
  "Please arrive at your appointed time.  The first 10-15 minutes of your visit will be with your PCMs nurse
        who will screen you for Preventive Medical needs, health risks and review your expectations for your visit."

Provider Restrictions
  Cox:  Float provider.    She ONLY sees patients assigned to Binder, Cason-Wilkerson, or Reich requesting 
                 Physical Exams in her afternoon EST appointment slots with detail code of PE.  Do not use these for other reasons.
 
  Coombes:  Does not see ADHD or refill requests for ADHD meds, Psych Evaluations, depression, or GYN

       If there are NO ACUTE slots available then a SAME DAY EST slot may be used.
       If a patient has a NON-ACUTE PROBLEM and REQUESTS a same day appointment, you may book them into a 20 min ACUTE with
       their PCM (20 min ONLY) if there are no available same day EST appointments.  (Not to be used for PAP or ADHD) 
 
 
  Siblings MAY be booked back to back with the SAME PROVIDER.  If siblings are booked with SEPARATE PROVIDERS,
      they MUST be booked no closer than 40 minutes apart, NO exceptions.
  Cunico-Lowery ONLY,   Please limit to only 2 family members that may be booked back to back.
 
Multiple Issues / Concerns
  Book ONLY one appointment per patient per day, list ALL concerns - do not join. 
DO NOT TELL THE PATIENT THERE ARE ANY LIMITATIONS OR RESTRICTIONS
 
Well Baby
  Newborn Check:
    ► Any newborn up to 5 days old - DO NOT BOOK,  transfer parent to the clinic RN:  225#      
     
  Well Baby appointments: ( 2 weeks*, 2 mos / 4 / 6 / 9 / 12 / 15 / 18 / 24 months) in the following priority: 
      1.  Wetzig or Cunico-Lowry (detail code of WB) See EACH Team Listing for providers team
                NOTE: if patient insists on PCM then book with the PCM.
      2.  PCM (No detail code)
      3.  PCM Team (No detail code)
      4.  Other Team (No detail code)
     
*Restriction on 2 week WB -If infant cannot be booked between 10-17 days of life, please transfer parent to the front desk.
 
Physical Exam Guidelines
 (School / Sports / Camp / Annual)
  Ask the parent if the child has been seen for a PHYSICAL within the previous 9 months.  If they have, do not book.  Transfer
      the call to the clinic:  225#.  Do not book under age 5 physicals in the 1530 EST appt.  
  Ages 2 - 17 years.
  If there are no Sports/Camp Physicals available, transfer to the clinic:  225#
  Read the script below for ALL School, Sports, Camp physicals:
Physical Script
  Your child is now booked for a sports/school physical on __(date)__ at __(time)__.  
 A parent or legal guardian must accompany children 17 and under.  In addition, all required documents  must be brought with you to
 the appointment.  This includes the school/sports physical form, your child’s shot record, and the child’s dependent I.D. card (if applicable).
 The Pediatric Clinic does not provide these forms.  Please complete the basic patient information and patient answer portions before arrival. 
 
Pre-Operative
  Pre-OP physicals are to be done with PCM in a 20 min appt.
General Clinic Information
 
Request for Referral
   Appointments are not needed for referral requests.  Transfer the parent to the clinic so they may leave a message with the PCM nurse.
 
Walk-In
   Staple/Suture Removal:
  Patients calling for staple removal may walk in from 0800-1130 and 1300-1600.
            Exception:  If staples were placed in the ER, patients must walk into the Carson Care Clinic for removal between 0800 and 1730.
The Carson Care Clinic is located in the Evans Family Medicine Clinic on the 2nd floor in the Soldier Family Care Center.
 
  Strep Testing / Sore Throat:
  If the patient is between 3 and 17 years of age, and the chief complaint is “Sore Throat” then DO NOT BOOK, instead recommend
      they come in on a Walk-In basis to Pediatrics from 08:00-16:00 for a STREP TEST.
 
  Immunizations:
  Patients up thru the age of 17 can Walk In to the Pediatric Immunization Clinic Monday thru Friday from 0730-1600.  
  Any patient enrolled to Evans FMC, DiRaimondo, Robinson, Premier, or Warrior Clinic must report to their PCMs clinic
      for immunizations. 
  Patients over age 17 needing immunizations must report to the Adult Immunization Clinic.
 
Communicable Diseases
 (Chicken pox, measles, etc.)
  Transfer to the clinic for appointment booking:  225#
 
Medications Refills / Renewals
  If the patient requires a prescription RENEWAL
       (ie. their prescription has expired or out of refills) transfer to clinic to leave a telephone consult for PCM.
 
  If the patient requires a prescription REFILL
       (their prescription has refills left) continue with the following:
          - Ask the patient to have their prescription bottle present with them.
          - Explain to the patient that you will now transfer them to the Med Refill Line where they may follow the prompts to obtain a refill.
          - Give the patient the phone number and attempt to transfer the patient to: 719-524-4081
 
 
EFMP (Exceptional Family Member Program)
   EFMP physicals, book with patient’s PCM.
   Requests for any other EFMP reasons, transfer to clinic:  225#
 
Child Abuse Evaluations
   MUST GO TO SOCIAL WORK SERVICES first.
   May walk -in Mon - Fri 0730-1630 at bldg 6236.
   For directions, transfer to Abuse Clinic:  526-4585
 
Circumcision
   All requests for circumcision must be transferred to clinic:  225#
 
Mental / Behavioral Health Requests
  Non-emergent requests for non-active duty Mental Health needs, transfer call to 1-888-TRIWEST, and instruct patient to say:
 'Mental Health'
