Clinic Location:   First floor of the hospital across from the main pharmacy.
  Enter the hospital through the revolving door, west entrance on the mountain side of the hospital.
  Parking on the west side of the hospital is most convenient (Parking lot A ).

  Children cannot be left unattended in the clinic waiting area.
  A list of all medications, prescription as well as over the counter, herbal supplements and vitamins must be brought to your appointment.
  "Please arrive at your appointed time.  Early arrival is not necessary.  The first 10-15 minutes of your visit will be with your PCMs nurse
        who will screen you for Preventive Medical needs, health risks and review your expectations for your visit."

Provider Restrictions
    Patients enrolled to Brewer  are to be booked with Dawson.  If no access, follow the appropriate booking priority.
    Wherley, Dawson, and Hughes:  Do not book for Pre-op Surgery or Consults / Referrals.
     Patients enrolled to Dr Tebo are to be booked with Dr Markusfeld.
    Sheridan, Griffin, and Delgado-Bryant:  They are NOT PCMs.  DO NOT BOOK  They are Coumadin Clinic providers.  
Appointments with these providers do NOT prevent  the patient from having an additional appointment  with an IMC provider.
Multiple Issues / Concerns
  Book ONLY one appointment per patient per day, list ALL concerns - do not JOIN appts (unless specified elsewhere).
  EXCEPTION to this process is PAP appts.  They should NOT be scheduled with any other appt requests.
  If a patient requests a PAP and Physical, there should be a MINIMUM of 2 hours between appts, or if possible, book on seperate days.
DO NOT TELL THE PATIENT THERE ARE ANY LIMITATIONS OR RESTRICTIONS
 
All Appointment Booking Priority
 
1.  PCM     (Same Day/Next Day - all appts)
2.  PCMs Team     (Same Day Only for ACUTE appts)
3.  Other Clinic Team     (Same Day Only for ACUTE appts)
 
       If there are NO ACUTE slots available then a SAME DAY EST slot may be used.
       If a patient has a NON-ACUTE PROBLEM and REQUESTS a same day appointment, you may book them into a 20 min ACUTE with
       their PCM (20 min ONLY) if there are no available same day EST appointments.
 
Appointing
  Follow-ups will be booked with their PCM.
  The goal is to have NO appointment slots go unfilled.
  Patients that are requesting only a med refill should be transferred to the clinic.
Active Duty TDY
  Active Duty personnel in a transit status, on leave, on Active Reserve Status, or visiting the Ft Carson area requesting an
      appointment may be booked into any available 20/30 minute appointment.  If there is no available access then they can be scheduled with Evans
      Family Medicine Clinic (EFMC).
  Active duty reserve and National Guard stationed at Buckley AFB, PAFB or the USAFA  in need of a permanent profile or profile
 review will  be transfered to DFMC (211#) to leave a T-Con for CPT Carmen Smith.
 
Physical Exam Guidelines
Active Duty
  Active Duty Physicals, Part 1 and II are to be booked with Evans Physical Exams Clinic (See protocol for instructions)
            EXCEPTION: PART II OF MEB IS BOOKED WITH PCM.
 
Pre-Operative
  Consults:  Book with PCM (if PCM is a physician).  If PCM is NOT a physician, book with PCM team physician.  
  Physicals:  Pre-OP physicals, if patient is enrolled to  IMC,  book in a 30 min EST with a physician on their PCM team only.
Female / Gyn Concerns
PAPs
  PAP appts should NOT be scheduled with any other appt requests.
  If a patient requests a PAP and Physical, there should be a MINIMUM of 2 hours between appts, or if possible, book on seperate days.
  All other Internal Medicine patients will be booked in the following priority:
      1.  Hughes AMHIMEACHTM2 in a PAP detail coded EST.
      2.  GYN (see Protocol for instructions)
NOTE:  PAP smears for patient under 21 years of age are rarely performed and will only be done at the discretion of the provider.  
   Patients will get a routine well woman exam with the omission of the PAP.
General Clinic Information
Medications Refills / Renewals
  If the patient requires a prescription RENEWAL
       (ie. their prescription has expired, is out of refills, or does not have the prescription bottle)
        Transfer to clinic to leave a telephone consult for PCM.
 
  If the patient requires a prescription REFILL
       (their prescription has refills left) continue with the following:
          - Ask the patient to have their prescription bottle present with them.
          - Explain to the patient that you will now transfer them to the Med Refill Line where they may follow the prompts to obtain a refill.
          - Give the patient the phone number and attempt to transfer the patient to: 719-524-4081
 
Coumadin
  WE DO NOT BOOK COUMADIN APPOINTMENTS.  Transfer patient to the clinic:  214#
 
EFMP (Exceptional Family Member Program)
  Book EFMP enrollment, updates, and disenrollment with PCM only.  Join two 20 min EST appointments.
  AD family members only - NO Active Duty.
  Patient needs to pick-up form DD2792 from the EFMP Office and bring it to the PCM appointment.  The form must be brought back
      to the EFMP office following the appointment.
  Overseas Clearance – Transfer to EFMP clinic:  237#
 
Mental / Behavioral Health Requests
  Non-emergent requests for non-active duty Mental Health needs, transfer call to 1-888-TRIWEST, and instruct patient to say:
 'Mental Health'
