TO BE READ VERBATIM TO THE PATIENT
   Do not bring children to pap appointments unless they are under the age of 6 weeks old.
   Children cannot be left unattended in the clinic waiting area.
  Patients under the age of 18 must be accompanied by a parent or legal guardian unless the appointment is for birth control or any
       GYN or OB issues to include STDs. 
    Plan on spending approximately one hour in the clinic.  
  "Please arrive at your appointed time.  Early arrival is not necessary.  The first 10-15 minutes of your visit will be with your PCMs nurse
        who will screen you for Preventive Medical needs, health risks and review your expectations for your visit."

Multiple Issues / Concerns
  If a patient has 3 or more concerns, join TWO back-to-back appts.
  EXCEPTION to this process is PAP appts.  They should NOT be scheduled with any other appt requests.
  If a patient requests a PAP and Physical, there should be a MINIMUM of 2 hours between appts, or if possible, book on seperate days.
DO NOT TELL THE PATIENT THERE ARE ANY LIMITATIONS OR RESTRICTIONS
 
ACUTE Booking Priority
  1.  PCM     (Same Day/Next Day)
  2.  PCMs Team     (Same Day Only)
  3.  If PCM or PCM team has no access:   Team 1 and 2 patients are to be cross-booked to each other (Same Day Only)
                                                                        Team 3 and 4 patients are to be cross-booked to each other (Same Day Only)
4.  Any Team     (This is LAST RESORT -Same Day Only)
     
       If there are NO ACUTE slots available then a SAME DAY EST slot may be used.
       If a patient has a NON-ACUTE PROBLEM and REQUESTS a same day appointment, you may book them into a 20 min ACUTE with
       their PCM (20 min ONLY) if there are no available same day EST appointments.
 
NON-ACUTE Booking Priority
1.  PCM     (Same Day/Next Day - all appts)
2.  PCMs Team    - Only if the PCM does NOT have access within 15 days and the patient requests a sooner appt.
     
If a patient has a NON-ACUTE problem and REQUESTS a same day appt, you may book them into a 20 min ACUTE with their PCM if there
   are no available same day EST appointments.
 
Appointing
  Put type of appointment in comment section, especially if appointment is a PAP, Well Baby or school physical so that the clinic staff can
       prep for appointment.
 
Active Duty TDY
  See instructions in E IMC protocol.
 
FORSCOM APPOINTING INSTRUCTIONS
  Instructions are found in the Evans FORSCOM Provider Instructions protocol.
     
Reserve Unit Personnel assigned to 1/158th Apache:
   IMC is the assigned TMC (Troop Medical Clinic) for this unit’s personnel and should be scheduled with IMC providers first, if there
       is no available access then they can be scheduled with Evans Family Medicine Clinic (EFMC).
               
Pre-Operative
  Book with PCM (if PCM is a physician).  If PCM is NOT a physician, book with PCM team physician.  
Well Baby
  Well baby EST appointments (2 weeks, 2 / 4 / 6 / 9 / 12 / 15 / 18 / 24 months). 
  Make all attempts to book with their PCM.
  Early discharge appointments are to be booked into a EST appt type.
  Infants MUST be 72-120 hours (3-5 days) of birth on the day of their scheduled appointment.
Physical Exam Guidelines
MILITARY, Active Duty, and MEB ( Med Eval Board )
  All Active Duty Physicals (ie annual, Special Forces, OCS, etc.) Part 1 are to be book with Evans Physical Exams Clinic
       (See that protocol for instructions. )
  Part II are to be booked with PCM.  For Part II of an MEB Physical, join two EST slots.
  If there is no access within the PCM clinic when booking a Part II, book in Physical Exams in a SPEC appointment.
         NON-MILITARY: Annual / School / Camp / Sports / Daycare
  ► No CDL, DOT,  or Workman's Comp physicals.
  Siblings MAY be booked back to back with the SAME PROVIDER.  If siblings are booked with SEPARATE PROVIDERS they MUST
       be booked no closer than 40 minutes apart - NO exceptions.
  Lab tests MAY be required by your provider.  Please FAST for 12 hours prior to your appointment.  Your PCM will inform you
  at the time of your appointment if or when to go to the lab.  
PHAs
  Patients calling to schedule a Periodic Health Assessment (PHA), should be directed to call the Fort Carson SRP Site at 719-526-2014.
Female / Gyn Concerns
Birth Control
  If patient is requesting to initiate Birth Control they must schedule an EST appointment with their PCM.
  An appointment is required for Birth Control Pill refills, additionally:
            - Patients under the age of 30 must book a PAP appt first if it has been longer than 12 months since their last PAP.   
            - Patients 30 years of age and older must book a PAP appt first if it has been longer than 36 months since their last PAP.
     
Colposcopy
  Transfer call to 719-526-7908 to leave message with PCM.  Clinic nurse will book appt.
     
OB (Obstetrics) Guidelines
    OB patients (assigned to OB/GYN or FMC for OB care) who require non OB related care can be scheduled with
       any Team provider.
    Patient must complete NPCL in EACH OB even though they are being cared for in EFMC.
  Routine OB (follow-up) care must be booked in EST with assigned OB provider.
  OB PE's (physical exam) are booked at 12 weeks gestation, with the provider providing their OB  care ONLY.
  If there is no access with the provider, look for access with C. Demichiei.
     
IUD (Intrauterine Device)
  IUD counseling - book with Tromara, C. Demichiei, Tanaka, Tyler, M. Demichiei, Keeler, and Heinz ONLY.
  Clinic team nurses will book IUD insertion and removal.
 
Implanon
  Implanon counseling - book with Tromara, Keeler, Tullberg, Wencl, Sockol-Guest, Tyler, or May ONLY.
  Clinic team nurses will book the procedure.
 
PAP/Post-partum PAP Booking
  PAP appts should NOT be scheduled with any other appt requests.
  If a patient requests a PAP and Physical, there should be a MINIMUM of 2 hours between appts, or if possible, book on seperate days.
* PAP appointments for patients age 40 or over are booked in TWO JOINED BACK-TO-BACK EST slots only.
Exception: C. Demichiei DOES NOT REQUIRE patients over 40 to have joined appointments.  
                                        Book only one appt for her patients - DO NOT JOIN.
 
  Book in the following priority:  (Unless the patient asks us (we do not ask them) for a female provider)
          1. Mrs. C. Demichiei (AMHFPEACHTM3) or PCM ( Patient preference)
          2. PCM Team
          3. Other clinic providers with access
NOTE:  PAP smears for patient under 21 years of age are rarely performed and will only be done at the discretion of the provider. 
          Patients will get a routine well woman exam with the omission of the PAP.
  PAP Appointments must be at least 365 days from the previous appointment unless patient has received a letter from the clinic indicating
      the need to be seen in 3 or 6 months.  In this case, the requirement would be 90 or 180  days.
General Clinic Information
Medications Renewals / Refills
  For Birth Control Pills, book EST appt if longer than 12 months since last appt in Family Medicine Clinic.
  For thyroid meds (e.g. Synthroid) book appt if longer than 11-12 months since last appt in Family Medicine Clinic.
 
  If the patient requires a prescription RENEWAL
       (ie. their prescription has expired or out of refills) book in an EST appointment.
 
  If the patient requires a prescription REFILL
       (their prescription has refills left) continue with the following:
          - Ask the patient to have their prescription bottle present with them.
          - Explain to the patient that you will now transfer them to the Med Refill Line where they may follow the prompts to obtain a refill.
          - Give the patient the phone number and attempt to transfer the patient to: 719-524-4081
 
Lab Requests
  For lab requests, transfer to clinic:  29#
Vasectomy
  Book only the EST with M. Demichiei for the initial consult, the clinic will book the procedure.  Or build referral to EACH Urology.
     
All other Procedures
  Book EST appt if patient has not been evaluated in FMC.  Nurse will book actual procedure appt.
     
Immunizations
  Patients enrolled to EFMC up to age 17 can walk-in to Room 1400 Lab/Immunizations on 1st floor of SFCC Mon-Fri 0730-1600.
  Patients enrolled to EFMC over age 17 can walk-in to Adult Immunizations (co-located with IMC).
     
TRICARE PLUS / ADD / ADHD Testing and Evaluation
  Tri-Care Plus patients need 40 minutes for initial visit.  Combine two appointment slots.
  Initial evaluation for Attention Deficit Hyperactivity Disorder (ADHD): book with PHYSICIAN only:
M. Demichiei ,  Hull ,  Lang ,  Tromara ,  Tullberg
     
EFMP Screening (Exceptional Family Member Program)
  Family member that the appointment is for MUST be present at the appointment.
  Each family member must have an individual appointment.  There are NO joint family appointments.
  Book EFMP enrollment, updates, and disenrollment with PCM only.  Join two EST appointments.
  AD family members only - NO Active Duty.
   Patient needs to pick-up DD form 2792 from the EFMP Office and bring to PCM appointment.  Form must be brought back to EFMP
       office following appointment.
   Requests for any other EFMP reasons – transfer to clinic at 524-4068.
     
Communicable Diseases (Chicken pox, measles, etc.)
  Instruct patients with potential communicable diseases, i.e. chicken pox, measles to identify themselves to the receptionist.
 
Mental / Behavioral Health Requests
  Non-emergent requests for non-active duty Mental Health needs, transfer call to 1-888-TRIWEST, and instruct patient to say:
