  Do not bring children to pap appointments unless they are under the age of 6 weeks old.
  Children cannot be left unattended in the clinic waiting area. 
  Patients under the age of 18 must be accompanied by a parent or legal guardian unless the appointment is for birth control,
      or any GYN or OB issues to include STDs.
  "Please arrive at your appointed time.  Early arrival is not necessary.  The first 10-15 minutes of your visit will be with your PCMs nurse
        who will screen you for Preventive Medical needs, health risks and review your expectations for your visit."

    Humbargar:  Only sees patients 2-64 yrs.
 
  Dependants are ONLY booked with Team #1 providers listed above.
  If the soldier’s PCM is NOT listed on Team #1 , see the Evans FORSCOM Provider Instruction Protocol.
     
Multiple Issues / Concerns
  Book ONLY one appointment per patient per day, list ALL concerns - do not join. 
  EXCEPTION to this process is PAP appts.  They should NOT be scheduled with any other appt requests.
  If a patient requests a PAP and Physical, there should be a MINIMUM of 2 hours between appts, or if possible, book on seperate days.
DO NOT TELL THE PATIENT THERE ARE ANY LIMITATIONS OR RESTRICTIONS
 
All Appointment Types Booking Priority
1.  PCM     (Same Day/Next Day - all appts)
2.  PCMs Team -  (Same Day Only for ACUTE appts)  
2. Other Team - Either clinic (Same Day Only for ACUTE appts)  Make sure to tell the patient which clinic their appointment is at.
       If there are NO ACUTE slots available then a SAME DAY EST slot may be used.
       If a patient has a NON-ACUTE PROBLEM and REQUESTS a same day appointment, you may book them into a 20 min ACUTE with
       their PCM (20 min ONLY) if there are no available same day EST appointments. (Not to be used for PAP or ADHD).
 
Pre-Operative
  Consults:  Book with PCM (if PCM is a physician).  If PCM is NOT a physician, book with PCM team physician.  
  Physicals:  Pre-OP physicals are to be done with PCM in a 40 min appt ( join two 20 appts).
Well Baby
 
  Well baby EST appointments (2 weeks, 2 / 4 / 6 / 9 / 12 / 15 / 18 / 24 months).
  Early discharges (3-5 days) Parent needs to call the clinic and speak to an RN who will facilitate the appointment. 
 
Physical Exam Guidelines
MILITARY, Active Duty, and MEB ( Med Eval Board )
  All Active Duty Physicals, Part 1 are to be booked with Evans Physical Exams Clinic  (See that protocol for instructions). 
  Part II are to be booked with PCM.  For Part II of an MEB Physical, join two EST slots.
  If there is no access within the PCM clinic when booking a Part II, book in Physical Exams in a SPEC appointment type.
 
         NON-MILITARY: Annual / School / Camp / Sports / Daycare
  Adult / annual / school / sports / daycare physicals book in any available EST appointment.
  Non-Active Duty physicals - Siblings MAY be booked back to back with the SAME PROVIDER.  If siblings are booked with
      SEPERATE PROVIDERS they MUST be booked no closer than 40 minutes apart, NO exceptions.
  Patients over the age of 65 requesting physicals require 40 minutes.  Join two EST slots.
PHAs
  Patients calling to schedule a Periodic Health Assessment (PHA), should be directed to call the Fort Carson SRP Site:  719-526-2014
 
Female / Gyn Concerns
Birth Control
  Patients requesting Implonon counseling are to be transferred to the clinic for t-con to be left for PCM to input consult to GYN clinic.
  Birth Control Pill Refills require a current pap smear on record (within the last year). 
  If the patient requires a Birth Control Rx refill prior to their scheduled PAP appointment refer them to the front desk to leave
       a telephone consult with their PCM.  
  Patients requesting to initiate birth control pills also need a current PAP smear on record (within one year).  
  If the patient has a current pap, the clinic will book appointment for birth control counseling and initiation.
  If the patient needs a PAP, book an appointment for PAP and initiation of birth control.  
 
OB (Obstetrics)
  See Evan's OB Clinic for guidance. (click for protocol)
 
IUD (Intrauterine Device)
  For IUD counseling book patient an EST appointment with:  Humbargar or Raleigh at DiRaimondo MAIN
  Patient must have current pap smear (within six months).
  If patient needs a pap, schedule a pap appointment (following the PAP guidelines below).  When booking a pap with 
       Humbargar an EST appointment also needs to be booked at least 2 - 3 weeks out following the PAP (test results need to come back).  
  IUD insertion will be booked by the clinic for a day other than the counseling. 
  Any provider may perform removals.
 
PAP/Post-partum PAP Booking
  PAP appts should NOT be scheduled with any other appt requests (except INITIATION of birth control).
  If a patient requests a PAP and Physical, there should be a MINIMUM of 2 hours between appts, or if possible, book on seperate days.
  PAP appts are 20 minutes unless patient is over 40 years of age, then book in TWO JOINED BACK-TO-BACK EST slots only. 
  All PAP appts should be booked in the following priority and following the appt guidance for clinic you are booking into:
NOTE:  PAP smears for patient under 21 years of age are rarely performed and will only be done at the discretion of the provider.  
   Patients will get a routine well woman exam with the omission of the PAP.
 
  1.  PCM – (See instructions above for duration)
  2.  GYN (click for protocol)

General Clinic Information
Medications Refills / Renewals
  If the patient requires a prescription RENEWAL
       (ie. their prescription has expired or out of refills) transfer to clinic to leave a telephone consult for PCM.
       If patient experiencing a medical problem/change, an appointment must be booked.
 
  If the patient requires a prescription REFILL
       (their prescription has refills left) continue with the following:
          - Ask the patient to have their prescription bottle present with them.
          - Explain to the patient that you will now transfer them to the Med Refill Line where they may follow the prompts to obtain a refill.
          - Give the patient the phone number and attempt to transfer the patient to: 719-524-4081
 
EFMP Screening (Exceptional Family Member Program)
  Book EFMP enrollment, updates, and disenrollment with PCM only.  Join two EST appointments.
  AD family members only - NO Active Duty.
  Patient needs to pick-up Form DD2792 from the EFMP Office and bring to PCM appointment.  Form must be brought back to the
      EFMP office following appointment.
  Requests for any other EFMP reasons - transfer to clinic:  210#
Lab Work
  Strep Test and STD, book any available appointment.
  Pregnancy Test, walk-in to DiRaimondo Clinic during normal business hours.
 
HPV Immunizations
  HPV injections are a series of three injections.  
  The first injection requires an appt.
  The two subsequent injections, at two months and again at six months are on a Walk-In basis.  
 
Communicable Diseases (Chicken pox, measles, etc.)
  Instruct patients with potential communicable diseases, i.e. chicken pox, measles to identify themselves to the receptionist.
 
Vasectomy
  Book the consultation in an EST with Raleigh.
  Clinic will book the procedure after the EST.
 
Mental / Behavioral Health Requests
  Non-emergent requests for non-active duty Mental Health needs, transfer call to 1-888-TRIWEST, and instruct patient to say:
 'Mental Health'
