Who we are:
The Mountain Post Medical Home is the second Army community-based primary care clinic in Colorado Springs. The clinic is located in Patriot Park just west of Powers Blvd and just off Platt Ave., at 565 Space Center Drive in the Patriot VII building.

The Mountain Post Medical Home operates on a patient-centered primary healthcare model using a team based, comprehensive approach to meet the patients' complete primary care and wellness needs. On-site services include family medicine, pediatrics, behavioral health, immunizations, laboratory services, and limited pharmacy services for your convenience.

How do you access your care?
If you feel that your condition is an emergency, please visit the nearest Emergency Room.

Telephone Consultation (TCON) Requests
Send a message to your care team through the Call Center at (719) 524-2273 (CARE) or 866-422-7391 to request a TCON Monday – Friday, 6 a.m. - 4:30 p.m. A member of your care team will call back with:

• Recommendations for basic self-care at home
• Prescription renewals for current medications Including gap renewals
• Test results, also at
• Medication answers
• Information about quitting tobacco
• Information about weight management
• Orders for lab tests required prior to your visit
• Paperwork reviews
• Referrals for colonoscopy
• Renewals for current referrals New referrals require an appointment
• Updates on weight, blood pressure or cholesterol management

Army Medicine Secure Messaging Powered by Relay Health
You can now send a secure email to your care team through Army Secure Messaging powered by Relay Health! Simply talk to a member of your health care team about getting registered. We will send you a personal invitation to begin sending secure messages to your care team at to:

• Schedule/cancel appointments
• Request/review lab and test results
• Request medication refills
• Request a referral
• Email questions to your care team
• Schedule web visits with your care team

After-hours Care
One of the great benefits of the Patient-Centered Medical Home model is the improved continuity of care. For the model to work effectively for you, it is important that your care team be involved in your care. Therefore we ask that you reserve Emergency Room visits for true emergencies.

If you have a non-emergency medical concern after hours, please call the TRICARE Nurse Advice Line at 1-800-TRICARE (874-2273), and select Option 1. Walk-In Services
Did you know that you can receive many health care services on a walk-in basis?
Walk-in services are available Monday through Friday from 8:30 a.m. to 11:00 a.m. and 1:30 p.m. to 3:00 p.m., except on the afternoon of the last Thursday of the month when walk-in services are from 8:30 until 11:00 to allow time for provider training in the afternoon. Please understand that walk-in services are subject to staffing and appointment levels and may result in variable wait times.

The Mountain Post Medical Home offers the following walk-in services
* Immunizations (patients 2 years old and older)
* Contraceptive management, including Depo-Provera shots when due, with a prescription (other than initial shot)
* Pregnancy Tests

Access to Care Standards
The medical home team makes every effort to contact patients with urgent and acute concerns within two hours, but will contact you within 24 business hours. In addition, we work hard to contact patients with more routine concerns by the end of the business day but will contact you within three business days.

Tricare Online
Tricare Online (TOL) provides secure access 24/7 to online features for DoD beneficiaries receiving care through a Military Treatment Facility (MTF). Register for TOL at to:
• Schedule/cancel appointments for yourself or your authorized Family members
• Get email and text message appointment reminders
• Refill and check the status of your prescriptions
• View or download your personal health data via the Blue Button, including your lab results, allergy profile, and medication profile
Want to change your enrollment to Mountain Post Medical Home?

Enrollment is open to all TRICARE Prime eligible beneficiaries 62 years old and younger that live within a 30 minute drive time of the clinic.

Eligible TRICARE Prime beneficiaries who are 62 years old and younger that live within a 30 minute drive time of the clinic may choose to switch to the Mountain Post Medical Home, or remain enrolled to their current military treatment facility healthcare provider.

Eligible beneficiaries who meet the above criteria that would like to change their enrollment to the Mountain Post Medical Home can do so by filling out and signing a Primary Care Manager (PCM) Change Request Form. The form may be  filled out (downloaded here), signed and mailed to the address below. Beneficiaries who want to make the PCM change over the phone or need assistance completing the PCM change request should contact 1-877-988-WEST (1-877-988-9378).
