 Case Management - Services
Services provided
  1. Help you coordinate care between your PCM and specialists.
  2. Help you and your family, make informed health care decisions.
  3. Help with coordination of needed home health care, therapy, medical equipment and/or nursing visits.
  4. Navigating the health care system can be difficult, frustrating, and frightening when you or a family member is very sick.  Our case managers are experts at helping you get through the system and get the best possible care in the process.
  5. Help you plug in to the local resources you need to manage your health and illness needs.
  6. Help to make sure that the care you or your family member is getting the best, most appropriate care for your situation.
  7. Work with your PCM to assure you or your family member receives the care needed, when it's needed.
  8. Evans Army Community Hospital case managers are here to advocate for YOU, our patient!
