1) Complete the 'Commander's Request for Mental Health Evaluation' and it must be signed by Unit Commander.  

FC65 Update

2) Fax completed form to Behavioral Health 719-526-7852.  For 4th ID Soldiers, take the FC-62 to your respective Mobile Behavioral Health Team.

3) Once the form is reviewed for correct completion, the Commander will be called for a future appointment for the evaluation.
