The Clinical Pharmacy section delivers direct patient care to patients with chronic conditions requiring extensive medication monitoring, adjustment and education. A pharmacist works in the disease management clinic adjusting pharmacotherapy for patients with Diabetes, Asthma, and COPD and educating them on their disease state and medications. The pharmacist working in the anticoagulation monitoring service monitors lab values of medications that prevent the blood from clotting. This pharmacist adjusts the dosing and schedules lab tests for patients assigned to this clinic. The refill extension service is a plan designed to benefit our providers in internal medicine clinic by extending prescriptions for 90 days if the criteria are met for a patient on a chronic medication.
